Neko Mulally took his personally designed downhill bike to the Costa Rica Open to put it under race conditions. Spoiler - the bike worked, Neko won. He chose to run the lower-pivot design. Check out some pics of the bike and Neko in action.

Anywhere from $3000 - $8000 for frame only(hell probably $10k+ depending on what kind of Sultan of Brunei shit you want), and you will be extremely limited should you want aluminum. Most custom frame builders are going to be steel, titanium, or carbon w/lugs. If you want your own design, linkage and everything, well...get a welder and some tools. In short, "more than you can afford pal!" - Ferrari driver from Fast and Furious. If you want a "custom"(need to be careful about what your calling custom) frame, get an Atherton Bike Co(formerly known as Robot Bike Co), they use a Dave Weagle designed linkage(DW6) and offer carbon frame with 3d printed titanium lugs and will offer any custom geo you like. Expensive, but IMO the "best"(see: coolest) custom frame you can get.

Neki said in his video the first frame cost him 10.000 USD. The cost driver was the heat treatment, where just starting up the oven is 2500 USD,regardless if it's for one frame or 50 frames. The rest covered the materials, the CNC work in a rapid prototype shop (never cheap as they usually specialize in quick turnarounds and the like and price accordingly), the modelling by an english engineer (came recommended by Frank) and the welding/production itself.

As far as 'custom' a-la csermonet goes, custom geometry is offered by a few brands, Nicolai and MDE included. But in neither of the cases you get a custom suspension design. Which was the name of the game here. Also, regarding 'coolest' frame, different strokes for different folks...
